• clinic and/or in-home service provision with outreach to all of southern NB

• Patient obtains a written referral which is to be forwarded to our office;
  
  
• Patient contacts our office at 648-6150 to send out pre-assessment questionnaire (if applicable to be completed by their Occupational Therapist);
  
  
• Once a completed questionnaire is received by our office, the patient will be contacted to set the initial assessment appointment;
  
  
• Upon completion of assessment, the OT will prepare a report and funding will be requested from appropriate sources;
  
  
• Once funding for the equipment is confirmed the equipment will be ordered through our office;

• Delivery of equipment to our facility may take up to 8 weeks.;

• Once equipment arrives it is adjusted and/or customized to meet predetermined needs;
  
  
• The Patient will be contacted to schedule a fitting appointment where the equipment will be adjusted to achieve an ideal fit.  The client will be provided with instructions & training on the use and care of the equipment;

• Follow-up will be provided to the client, as required, for the life of the equipment.
